Effective May, 4, 2026, the process for Veterans to request medical information from VA for a form has changed. This change will help VA protect the personal health information of Veterans, simplify the process for requesting medical information, and ensure policy and legal compliance.
To request VA support with completing medical documentation for forms, please contact the VA Long Beach Release of Information (ROI) Office by calling 562-826-8000, ext. 16345, or visiting their office in Building 165, Room 108, at the Tibor Rubin VA Medical Center. You may also Secure Message them using My HealtheVet by contacting LB: Release of Information_Admin.
Common forms that apply for this new process include: state disability, life insurance, disability placard, FMLA, and reasonable accommodations.
Please note that forms may require up to 20 business days to process after required paperwork and appointments are completed. You will need to schedule an appointment with your provider if 1) you have not been seen for this diagnosis within the last 3 months, or 2) this request is for a condition not previously discussed at your prior appointment.
Do you want to try something different for pain management? Battlefield Acupuncture (BFA) is an Auricular (Ear) Therapy that can assist in pain relief, and in many cases, eliminate the need for pain medication for both acute and chronic pain.
