Your VA Experiences Matter to Us

Share Your Feedback

Veterans may receive one or more surveys from the VA following their visit to their healthcare team or service. The surveys are sent randomly to Veterans following their visit and completion of the survey is voluntary.

The Survey of Health Experiences of Patients (SHEP) measures the overall experience with the care and services received. It is mailed to the Veteran through the U.S. Postal Service approximately one to two weeks after the VA visit. The survey was streamlined in January 2026 to make it easier for Veterans to complete. 

The Veteran Signal (VSignal) Survey is a shorter, electronic survey sent by text message or email, usually within one week of the Veterans visit. The questions vary based on the type of service received. The VSignal survey uses a Likert scale from 1 to 5, with 1 meaning poor and 5 meaning excellent. A comment section is provided allowing the Veteran to share a compliment or feedback about the care and services received during the visit.

Veterans may indicate on the surveys that they can be contacted, allowing a VA staff member to follow up to discuss the comment and take appropriate action to resolve the matter.

Veterans are encouraged to complete these surveys, as that the feedback helps us improve the VA experience and enhance the quality of care and services we provide to all Veterans.

Don't Forget to Schedule Your Annual Check-Up

If you haven't done so, please make sure to schedule your annual check-up with your VA health care provider as soon as possible. Annual check-ups provide an opportunity to engage and connect with the health care team, detect and treat health care conditions early before they worsen, and review health care concerns, goals, and what matters most to you. Many illnesses and diseases can be managed effectively if caught early. Make your health a priority!
