The CORE Outbreak Investigation Table is a list of outbreak and adverse event investigations primarily being managed by FDA’s CORE Response Teams. The investigations are in a variety of stages, meaning that some outbreaks have limited information, and others may be near completion. The table will be updated weekly.

What's New
A new outbreak of Salmonella I 4,[5],12:i:- (ref #1402) has been added to the table. FDA has initiated traceback.
For the outbreak of Cyclospora (ref #1398) linked to a not yet identified product, the case count has adjusted from 10 to 11 cases.
For the outbreak of Cyclospora (ref #1392) linked to a not yet identified product, the case count has been adjusted from 115 to 172. This incident now includes additional illnesses based on similarities in reported exposures.
For the outbreak of Salmonella Javiana (ref #1395) linked to jalapeño peppers, FDA updated the outbreak advisory on August 10, 2026, with information about recalled jalapeños. See the advisory for additional details. FDA has initiated sampling.
For the outbreak of Cyclospora (ref #1390) linked to iceberg lettuce, FDA updated the outbreak advisory on August 5, 2026, with case count information. FDA initiated an inspection.
For the outbreak of Cyclospora (ref #1385) linked to a not yet identified product, FDA has initiated sampling.
For the outbreak of Salmonella Oranienburg (ref #1387) linked to a not yet identified product, the case count increased from 93 to 97.
For the outbreak of E. coli O145:H28 (ref #1389) linked to frozen blueberries, FDA has initiated an inspection.
