The CORE Outbreak Investigation Table is a list of outbreak and adverse event investigations primarily being managed by FDA’s CORE Response Teams. The investigations are in a variety of stages, meaning that some outbreaks have limited information, and others may be near completion. The table will be updated weekly.

What's New
For the outbreaks of E. coli O103:H25 (ref #1394) and E. coli O26:H11 (ref #1404), these outbreaks have been merged into a single incident under ref #1394, based on FDA traceback findings and epidemiological data provided by the Centers for Disease Control and Prevention (CDC). Among the cases, CDC has identified four sick people who were infected simultaneously with multiple pathogens - three E. coli strains (O26:H11, O103:H25, and O168:H8) and Salmonella Agona, and has combined all associated clusters into one outbreak based on their epidemiological links. On August 21, 2026, FDA posted an outbreak advisory identifying alfalfa sprouts distributed by Everything Sprouts, LLC of Minneapolis, MN, as a source of illness in this outbreak. Additionally, FDA has initiated an onsite inspection and sampling. The outbreak advisory was updated on August 24, 2026, with a recall from Everything Sprouts, LLC. Refer to the outbreak advisory for additional information.
A new outbreak of Salmonella Newport (ref #1403) linked to a not yet identified product has been added to the table. FDA has initiated traceback.
For the outbreak of Cyclospora (ref #1398) linked to a not yet identified product, the case count has been adjusted from 17 to 22 cases.
For the outbreak of Cyclospora (ref #1392) linked to a not yet identified product, the case count has been adjusted from 201 to 203.
For the outbreak of Salmonella Javiana (ref #1395) linked to jalapeño peppers, FDA updated the outbreak advisory with information about product recalls related to this outbreak. See the advisory for additional details.
For the outbreak of Cyclospora (ref #1390) linked to iceberg lettuce, FDA updated the outbreak advisory on August 20, 2026, with case count information.
For the outbreak of Cyclospora (ref #1385) linked to a not yet identified product, the case count has increased from 27 to 28.
For the outbreak of Listeria monocytogenes (ref #1380) linked to Requeson/Soft Ricotta Cheese, the outbreak has ended. FDA updated the outbreak advisory with final investigational information on August 26, 2026.
For the outbreak of Salmonella Typhimurium (ref #1377) linked to moringa leaf powder, the Executive Incident Summary Abstract can now be accessed by clicking the reference number. This outbreak investigation was moved to the Closed 2026 Table.
